    DH Holistic Care is a family-oriented organisation who pride itself on providing quality of home and community base services. As part of our service continuous improvement, we want to hear from you. To ensure we continue to provide you with the right care to stay safe and comfortable within your home.

    • Survey can be completed by Clients or Client Representatives individually or by interview with a staff member – either in person or on the phone.

    • We encourage All Clients to have a ‘SAY.’ We reassure you that all your information or concerns will be kept private.

    • All Clients or Client Representatives are encouraged to provide comments in relation to the reason for their satisfaction/dissatisfaction.
