    DDH Holistic Care promote a healthy working culture to continue to support our hard-working staff. We respect that you might want your responses to be confidential, however, the aim of this survey is to fully understand where and how we can improve our working environment for all employees. Allowing us to see who completed our survey will enable us to make better informed decisions and resolve existing problems more effectively.

    Survey Notes:

    • Survey can be completed by Client's or Client Representatives individually or by interview with a staff member – either in person or on the phone.

    • We encourage All Clients to have a “SAY” and we reassure of their privacy when we address their concerns.

    • All Clients or Client Representatives are encouraged to provide comments in relation to the reason for their satisfaction/dissatisfaction.
