What is In-Home Care service.

In-home care support is to enhance the independence and health outcomes of an eligible person by avoiding early admission to hospital and/or residential care by providing flexible and accessible services to meet an entitled person’s assed clinical and/or personal care needs. 

Personal Care: grooming such as Bathing or dressing

Assisting a person by grounding and orienting them

Keeping a safe and comfortable environment.

Mobility assistance: Moving around, getting in and out of bed or shower

Helping with morning, and evening routines. Running errands such as shopping appointments, or meal preparation

Medical assistance: such as medication prompts, GP or Hospital appointment
